This Site is hosted on a secure cloud server in British Columbia, and is a collaborative effort between The Centre for Health Evaluation and Outcome Sciences (CHEOS) and Addictions and Concurrent Disorders (ACD) Research Group in the Department of Psychiatry at the University of British Columbia (UBC). In addition, the Government of Canada has generously funded the Site through Health Canada research funds in an agreement with UBC’s Faculty of Medicine, CHEOS, and ACD, which are collectively referred to herein as “us” or “we”. Privacy and Collection/Use of Personal Information We have adopted this Privacy Policy to explain what information may be collected on the Site, how we use this information, and under what circumstances we may disclose the information to third parties. We respect your privacy and are committed to protecting personally identifiable information you may provide us through the Site unless you consent to its disclosure. Cookies – In a similar fashion as other websites, the Site uses “cookies” to improve your experience on the Site and to collect anonymous information about how you use the Site. This information is compiled into anonymous traffic analytics, and is not available to the general public. However, cookies are not required for use of the Site. Account information and Activities – We keep a log of the IP addresses of non-registered users and members of the public who visit the Site. For registered users. We keep a log of IP addresses, a record of their respective logins and logouts, and activities on the Site by registered users. For all users creating a User Account (as herein defined), we request mandatory information such as an email address and full name. Optional information provided by a registered user may include age, gender, country and city of residence. Email addresses, full names, age, gender, City and Country is information that will not be publicly available to other users of the Site. No personally identifiable information will be made public through the creation of a User Account. The only public information available about the registered user on the Site will be a username selected by the registered user and the content posted on any public forums on the Site. Once a user creates or edits content, the contribution, time/date, and username are recorded and may be available to the public indefinitely via certain features such as Art with a meaning. Deleting text does not delete the record of the contribution, and previous versions of articles are available indefinitely. At your request, you may request the Site administrator at ramp@cheos.ubc.ca to cancel/disable your User Account. When cancelling a User Account, the site administrator, at its sole discretion, has the following options: Disable the account and keep its content. (default) Disable the account and label its content as belonging to the site’s generic username “Anonymous.” Disable the account and unpublish its content. Disable the account and delete its content. By disclosing your contact information or any other personally identifiable information available to the Site, you are consenting to the disclosure of this information to authorized personnel in UBC and its respective third-party service providers, some of which are located outside Canada, for purposes connected with administering the Site. We do not make this information available to anyone else unless we are legally authorized to do so. In addition to the above, we collect certain anonymous (non-personally identifiable) information to help us improve the Site and to evaluate the access, use and the impact of the Site’s materials and the Site: We collect information you provide about your use of and satisfaction with the Site through emails you send us and through the Site surveys, whether or not you voluntarily include your contact information. Occasionally, we report quoted feedback from users. We do not attribute feedback to specific individuals unless we obtain permission to use that person’s name along with the feedback When we report information about the Site’s access, use, and impact, we report aggregate, non-personally identifiable data. Through features like LifeChart and other basic user information, user data will be collected such as journal entries, HRA and other assessment scores. Data related to billing or payments will NOT be collected What data will be shared User profiles will not be personally identifiable by other parties All shared data will be aggregated and anonymous to protect privacy and prevent tracking of the user Demographic information will be shared anonymously for research and policy making purposes. No personally identifiable information will be sold, traded or shared with outside parties. Personal health information (eg. assessment scores, etc.) will not be associated with any government ID or Personal Health Number unless agreed with the user in a separate consent form. Patient data sharing with healthcare professionals will not be implemented, although it can be an option if an organization wishes to implement RAMP in their treatment facilities. How data will be used To improve user experience and the platform’s predictive ability: input information will be available for the user themselves to use, and for the development team to improve the system’s performance. Anonymous information will allow us to improve the site by evaluating the use and impact of the site’s materials. For research: The data will be used as a tool for ongoing research to understand risk factors and how different treatment modules are able to reduce and manage the risk Keeping Your Information Secure – We have implemented security measures we consider reasonable and appropriate to protect against the loss, misuse and alteration of the information under our control. Please be advised, however, that while we strive to protect your personally identifiable information and privacy, we cannot guarantee or warrant the security of any information you disclose or transmit to us online and we are not responsible for the theft, destruction, or inadvertent disclosure of your personally identifiable information. Use of Public Forums – We may offer blogs, message boards, bulletin boards, or similar public forums where you and other users of the Site can communicate.
